Bi, PASQUALE E. PEIIILLIE T HE ALKALINE PHOSPHATASE ACTIVITY of the granulocytes (LAP) in patients with chronic granulocytic leukemia (CGL) usually is not increased following stimulation by either adrenocortical hormones or pyrogen, whereas the LAP of normal subjects and most patients with nonleukemic disorders shows a definite rise following similar stimulation. ’ An occasional patient with well-documented CCL, however, will demonstrate an increased LAP during acquired infection or following either steroid or pvrogen administration.4 In nonleukemic subjects it has not been possible to determine with certainty the mechanisms leading to the rise in LAP which follows either acquired infections or the administration of corticosteroid or pyrogen. It is not clear whether the rise in LAP is due to the enhancement of enzyme synthesis by the mature circulating cells, or whether such stimuli exert their prime effect on the less mature marrow granulocyte precursors with subsequent delivery Of such cells into the circulation.’2 It has been suggested that the rise in LAP following pyrogen stimulation may be due to an initial selective sequestration of enzyme rich circulating cells followed by a sul)sequent redelivery of such cells into the circulation.7 In addition to these basic prol)lems it is uncertain whethier in CCL the appearance of granulocytes with increased LAP denotes the presence of a dual population of normal and leukemic cells or represents a temporary alteration in an aberrant biochemical process of the leukemic cell.25 This report concerns further attempts to elucidate the mechanism of LAP change in the granulocytes of a patient with CCL who transiently increased his LAP concomitant with an acute bacterial infection. Following recovery from the infection, gram negative endotoxin was injected in order to restimulate LAP. A histochemical method for demonstrating LAP seemed most suitable for the proposed study since it was felt that the appearance of granulocytes with increased LAP would serve as unique natural markers in the setting of the reduced enzyme activity present imi the patient’s granulocytes prior to stimulation.

Bullous eruption and elevated leukocyte alkaline phosphatase in the course of busulfan-treated chronic granulocytic leukemia.

IHRONIC GRANULOCYTIC LEUKEMIA is characterized by hepatosplenomegaly, a markedly elevated white blood cell count with a spectrum of immature and mature granulocytic elements present, a proliferative granulocytic bone marrow, low levels or absence of leukocyte alkaline phosphatase activity and the presence of the Philadelphia chromosome.’ The patient to be described had chronic granulocytic leuk...

Studies of Leukocyte Alkaline Phosphatase in Mongolism: a Possible Chromosome Marker.

I N 1959 LEJEUNE, GAUTHIER AND TURPIN’ reported the finding of 47 chromosomes, one more than the normal number, in the cells of children with mongolian idiocy ( Down’s syndrome ) . The extra chromosome, one of the small acrocentrics-prohablv number 21-has since been universally verified. Nowell and Hungerford,2 in 1960, showed that chronic granulocytic leukemia is also associated with an abnorm...

Leukocyte drumsticks in chronic granulocytic leukemia and related disorders.

I N PREVIOUS REPORTS we demonstrated that leukocyte alkaline phosphatase and catalase concentrations were altered in leukemia.9’21 In chronic granulocytic leukemia ( CGL), leukocyte alkaline phosphatase is low and catalase is high. These abnormal values return to normal in most patients with CCL in response to treatment. These enzyme changes were noted in the early stages of this disease at tim...

Chronic Granulocytic Leukemia Complicated by Ulcerative Colitis: Elevated Leukocyte Alkaline Phosphatase and Possible Modifier Gene Deletion.

EUKOCYTE ALKALINE PHOSPHATASE ( LAP) determinations have proved of clinical value in the differential diagnosis of certain myeloproliferative disorders. Low or absent enzyme activity of mature neutrophils is characteristic of chronic granulocytic leukemia, in contradistinction to the usual high levels in polycythemia vera and myelofibrosis with myeloid metaplasia.13 There are also a number of o...

F OR A NUMBER OF YEARS this laboratory has been interested in the phosphatase activity of isolated human leukocytes and the marked lability of leukocyte alkaline phosphatase in disease.1 3 Leukocyte alkaline phosphatase has been demonstrated to be essentially limited to the granulocytic cells and to be characteristically low in chronic myelocytic leukemia. Elevated values are noted in polycythe...
